COLLINSWORTH- Mr. Chad Wayne Collinsworth age 35 of Tom Nall Road Lenox, Alabama passed away suddenly Friday night April 27, 2012 after injuries sustained in a automobile accident. He was a native of Pensacola, Florida, A former resident of Pace, Florida, before moving to Alabama in 1991. Mr. Collinsworth was of the Baptist Faith, A 1995 Graduate of T.R. Miller High School, and a was employed at T.R. Miller Mill Company as a Stacker Machine Operator.

Survivors: Mother & Step-Father- Charlene & Terry Jackson of Lenox, Alabama Father- Wayne Collinsworth of Mississippi (1) Son- Eli Collinsworth of Lenox, Alabama (1) Daughter- Faith Collinsworth of Lenox, Alabama (1) Brother- Michael Collinsworth of Andalusia, Alabama Paternal Grandfather- Leroy Jackson of Pensacola, Florida Fiancee- Mischelle Price of Lenox, Alabama

Visitation will be held Sunday night April 29, 2012 from 6 until 8:00 P.M. at WILLIAMS MEMORIAL CHAPEL FUNERAL HOME BREWTON, ALABAMA.

Funeral Services are scheduled for Monday afternoon April 30, 2012 at 2:00 P.M. at WILLIAMS MEMORIAL CHAPEL FUNERAL HOME with Rev. Mike Skipper officiating. Interment will follow in Damascus Cemetery Brewton, Alabama with WILLIAMS MEMORIAL CHAPEL FUNERAL HOME OF BREWTON, ALABAMA DIRECTING.
